How can I get the username and password for my laboratory? If you have any difficulties gaining access to the pre-populated survey for your laboratory, please contact: Dr. Radboud Duintjer Tebbens.
What do you include in the cost categories? Please refer to the glossary to find definitions of survey terms.
Why does the survey not change pages when clicking "Next" or "Prev" or how should we respond to an error message? For some of the questions, the survey checks for a valid response (for example, see the information about Question 4 below) and the survey gives an error message if the entry is not formatted as expected. The error message should provide an indication of the requirement for a valid response, but if you have any questions, then please contact us.
Why does question 4 require the responses to sum to 100? This question asks for you to divide the total polio-supported staff time in your laboratory according to the different activities you conduct. Please note that you will need to use only numbers for this question, which means using a 0 (zero) instead of "None" or "Unknown" or "Not applicable." If you want to report a range for any of the entries to reflect uncertainty, then please include the best estimates that sum to 100 in Question 4 and provide details of the ranges for all of the entries with ranges in Question 9.
